Supported Living at Achieva

Supported Living is a housing option for people with disabilities requiring less than 30 hours of supervision and support per week. This program offers people who have progressed to a higher level of independence and assistance needed to navigate successfully in the community. Program staff will assist with locating affordable housing in the community of their choice and ensuring that the individual has safe and secure housing.

Once housing has been secured, the assessment process begins to determine the level of assistance needed to achieve the individual's identified outcomes. Program staff then work with the person to develop a plan to assist the individual with achieving those goals. At this time, the estimated number of hours of support needed by the individual is determined.

Individuals receive instruction and assistance with activities of daily living, such as, housekeeping, budgeting, laundry, meal preparation, grocery shopping and other skills needed to successfully live in the community. The Supported Living program also encourages and assists people in becoming active members of their communities. Staff provide assistance and oversight to ensure that individuals receive annual medical, psychiatric and dental examinations and follow-up care, as needed. Assistance is also provided to individuals in all other areas of their everyday lives.


How Are These Supports Funded?

Funding is allocated through the Federal Title XIX waiver program. An individual must be eligible for and receive funding from this program to qualify for supportive lifestyle support. Private pay options are also available.
